Whenever you have recurring problems like lower back or joint pain you should definitely see a doctor or a physical therapist before starting off on any exercise program (the same ought to be true for everyone, not just those with obvious aches and pains, but DEFINITELY for somebody like you). Your doctor or therapist will be able to suggest the exercises that are safe for you to perform.
In general however, swimming is probably the least impact form of cardio which also happens to be highly effective and I would definitely consider swimming as top cardio choice for somebody with lower back and joint problems. Biking is another relatively low-impact form of cardio which will be eaiser on the knees and back (just don't lean forward when riding or bike uphill too much). Walking should be OK as well. All sorts of running motions or stair climbing can aggravate lower back and the knee joint, so I'd try and avoid those.
